(CDM) solution for one of the New York hospitals. This mHealth application will help the hospital to manage their
chronic disease patients more effectively, thereby improving the quality of life of such patients.
The CDM solution presented in an international healthcare conference last week received excellent response from
the industry leaders. The application also got approved by IRB as a best practice/QI tool to be used in the IBD
department of the New York hospital.
The mobile solution will help patients with chronic disease interact more with their care provider (in between
visits) by providing necessary information related to the symptoms, medications & quality of life frequently. The
solution will store current & historical records related to the disease and patient health which can be easily
shared /emailed to multiple care providers.
